ADHD EVALUATIONS
Our evaluations are completed by a master’s level counselor who works with our medication providers to conduct an evaluation that is intended to help identify symptoms consistent with Attention-Deficit/Hyperactivity Disorder (ADHD) in adults. We take a collaborative approach that considers your personal history, current challenges, strengths, and overall functioning.

Investment: $1250.00
Tier 2 is designed for individuals who require more extensive documentation for academic, workplace, or personal needs and want a detailed understanding of how ADHD impacts multiple areas of functioning.

Insurance and Payment​
ADHD evaluations at Sage Psychiatry PLLC are offered as private-pay services and are not billed directly to insurance. ADHD evaluations are typically considered diagnostic or educational assessment services and are generally not covered by health insurance plans. This allows for a comprehensive individualized assessment process that is guided by clinical needs rather than insurance requirements. By not being restricted by third-party billing rules, we are able to use flexible assessment methods, appropriate documentation formats, and the time necessary to fully understand each client’s concerns and provide meaningful and clinically informed recommendations.

To make the process more manageable, payment is divided into installments over the course of the evaluation, unless other arrangements are made ahead of time.​

Accomodation Requests
ADHD evaluations are conducted within the scope of practice of a Master's level Colorado Licensed Professional Counselor and are intended to provide clinical diagnostic impressions and treatment recommendations. Acceptance of evaluation results for academic, workplace, or testing accommodations is determined by the requesting institution.

Add on Options and additional fees:
Additional services may be needed depending on the depth and complexity of the evaluation. To ensure transparency and flexibility, the following add-on options are available:

Tier 1 Upgrade to Comprehensive Report: If you decide within 90 days of receiving your Tier 1 summary report that you would like a comprehensive report, you may upgrade for $650.

Extended Clinical Time for Complex Evaluations: If an evaluation requires additional clinical time due to complexity, history, or further assessment needs beyond the standard evaluation process, this time will be billed at $150 per hour.

Additional Report Writing Time (Tier 2): If additional documentation time is required beyond the included report writing hours in Tier 2 due to case complexity or expanded documentation needs, this will be billed at $50 per 30-minute increment.

These options allow for appropriate clinical flexibility while maintaining clarity and transparency regarding services and billing.
