Tier 1: ADHD Evaluation with Summary Report

Investment: $600

Tier 1 evaluations is ideal for individuals seeking diagnostic clarification and practical recommendations for personal use, treatment planning, or discussions with healthcare providers.

Tier 1 includes: 

• Comprehensive clinical interview

• Review of developmental, educational, occupational, and mental health history

• Standardized ADHD assessment measures

• Screening for common co-occurring concerns

• Feedback session to review results

• Brief written summary report (1-2 pages) includes:

  • Reason for referral

  • Evaluation methods used

  • Diagnostic findings

  • Strengths and areas of challenge

  • Personalized recommendations

Tier 2: Comprehensive ADHD Evaluation with Full Report

Investment: $1250.00

Tier 2 is designed for individuals who require more extensive documentation for academic, workplace, or personal needs and want a detailed understanding of how ADHD impacts multiple areas of functioning.

Includes:  

• Everything included in the Summary Evaluation

• Extended review and integration of assessment data

• Comprehensive written report (8–15+ pages) includes:

  • Referral concerns and presenting symptoms

  • Relevant developmental, educational, occupational, and psychosocial history

  • Assessment measures and interpretation

  • Diagnostic formulation

  • Functional impact across multiple settings

  • Strengths-based clinical conceptualization

  • Detailed recommendations for home, school, work, and treatment planning

  • Documentation that may support accommodation requests when appropriate

• Detailed discussion of symptom presentation and functional impact

• Consideration of differential diagnoses and co-occurring conditions

• Detailed recommendations for treatment, accommodations, and support strategies

• Feedback session to review findings and answer questions

Insurance and Payment​

ADHD evaluations at Sage Psychiatry PLLC are offered as private-pay services and are not billed directly to insurance. ADHD evaluations are typically considered diagnostic or educational assessment services and are generally not covered by health insurance plans. This allows for a comprehensive individualized assessment process that is guided by clinical needs rather than insurance requirements. By not being restricted by third-party billing rules, we are able to use flexible assessment methods, appropriate documentation formats, and the time necessary to fully understand each client’s concerns and provide meaningful and clinically informed recommendations.

To make the process more manageable, payment is divided into installments over the course of the evaluation, unless other arrangements are made ahead of time.​

Accomodation Requests

ADHD evaluations are conducted within the scope of practice of a Master's level Colorado Licensed Professional Counselor and are intended to provide clinical diagnostic impressions and treatment recommendations. Acceptance of evaluation results for academic, workplace, or testing accommodations is determined by the requesting institution.

Add on Options and additional fees:

Additional services may be needed depending on the depth and complexity of the evaluation. To ensure transparency and flexibility, the following add-on options are available:

​

• Tier 1 Upgrade to Comprehensive Report: If you decide within 90 days of receiving your Tier 1 summary report that you would like a comprehensive report, you may upgrade for $650.

• Extended Clinical Time for Complex Evaluations: If an evaluation requires additional clinical time due to complexity, history, or further assessment needs beyond the standard evaluation process, this time will be billed at $150 per hour.

• Additional Report Writing Time (Tier 2): If additional documentation time is required beyond the included report writing hours in Tier 2 due to case complexity or expanded documentation needs, this will be billed at $50 per 30-minute increment.

These options allow for appropriate clinical flexibility while maintaining clarity and transparency regarding services and billing.
